Responsibilities Utilizes strong clinical and social expertise to assess complex patient care needs, coordinate care across the continuum, and engage with patients/families to ensure safe, timely patient discharge plans to the next level of care. Serves as an expert resource for complex cases and acts as a consultant to other care team members regarding clinical, psychosocial, and resource needs. Provides comprehensive bio-psychosocial assessments for complex patients (e.g., high-cost, long stay, and/or difficult to place) to include timely and appropriate planning to advance the discharge plan and ultimately reduce length of stay. Maintains a smaller caseload of select complex patients, as specified by hospital criteria, that requires increased attention and intensive care coordination due to discharge barriers. Monitors hospital patient census to identify early intervention opportunities where multiple levels of discharge barriers are present. Serves as a specialist on issues related to complex psychosocial and discharge needs, end-of-life care planning, resource needs, etc. Provides resource information necessary to aid patients/families in decision making and discharge planning. Partners with and serves as an expert resource to other social work staff and interdisciplinary team members concerning complex social determinants of health, situational dynamics, financial, legal, and social needs. Assists Social Work Director in establishing criteria and implementing policies related to complex case identification and discharge planning. Incorporates continuous process improvements to reduce length of stay and optimize financial metrics. Documents social work interventions accurately, consistent with departmental guidelines for content and timeliness. Participates effectively in interdisciplinary plan of care, including communication with physicians and staff, and participation in complex case meetings with hospital leadership. Performs other duties as assigned. Requirements Must possess a degree in Social Work MSW with 2 years post graduate experience in health care setting Fluency in Spanish preferred. Minimum of LMSW or LCSW.
