Under the direction of the Manager, Director of Orthopedic Services, and Physician Director, the Nurse Coordinator works collaboratively with physician leaders, physicians, and staff of the Orthopedic program to ensure goals and objectives are successfully met. The primary role of the Nurse Coordinator is to maintain communication with patients, families, staff, and providers in order to ensure coordination of patient care across the continuum. Clinical Practice 1.1 Manages incoming referrals, coordinates initial patient contact, and compiles list of presented patients in collaboration with attending physicians, fellows, and Advanced Practice Provider, (APP's). 2. Clinical Program 2.1 Assists in the coordination of the Orthopedic program. 3. Patient Safety, Quality, and Research 3.1 Creates and supports a culture and environment of safety. 4. Professional Development and Education 4.1 Participates in enhancing the professional development of the nursing staff and other health care providers involved in the care of Orthopedic patients. Qualifications EDUCATION Bachelor's Degree in Nursing required. Connecticut RN license EXPERIENCE A minimum of three (3) years experience in adult orthopedic acute care inpatient services, ambulatory or critical care. LICENSURE RN license in the State of Connecticut SPECIAL SKILLS Knowledge of orthopedic procedures and surgeries. Excellent collaborative, communication, and interpersonal skills required. Ability to triage non-urgent and emergent situations. YNHHS Requisition ID 173803 1.
