Job Description:

Completes comprehensive individualized client assessments, including relevant medical history and diabetes-related knowledge, skills, and behaviors. Develops and implements education plans with specific learning objectives tailored to individual client needs. Teaches clients in both group and individual settings using department curricula and various teaching techniques. Evaluates client progress towards achieving learning objectives and behavior change goals. Assists clients in identifying effective behavior changes and setting achievable goals for diabetes self-management. Coaches clients through the behavior change process to improve diabetes self-management. Documents client encounters accurately and timely. Communicates client progress assessments to referring physicians at scheduled intervals and as needed. Collaborates with multidisciplinary team members to address assessed client needs. Manages a multiple-client caseload with tact and discretion in handling confidential material. Participates in the development and continuous quality improvement of the education program. Evaluates clients' glycemic control using lab results and self-monitored glucose tests, recommending behavior changes to reach glycemic targets. Provides diabetes education via Telehealth if applicable Other duties as assigned.Knowledge, Skills, and Abilities:

• Associate's of Nursing [Required]
• Bachelor's of Nursing [Preferred]

Field of Study:
• Nursing

Work Experience:
• 2+ nursing experience [Required]

Additional Information:

Licenses and Certifications:
• Registered Nurse (RN) [Required]
• Basic Life Support - CPR Cert (BLS) [Required]
• Certified Diabetes Care and Education Specialist (CDCES) [Preferred]
