Summary: The Licensed Vocational Nurse (LVN) uses sound medical and professional judgment and best practices to support the medical, safety and well-being of every resident while maintaining compliance with established state nursing and licensing regulatory agencies, and federal standards requirements. The LVN will be responsible for managing medical care of their designated residents to include, but not limited to well visits, immunizations, sick visits, documentation, medication audits, supply inventory, ordering and communication with parents/guardians.

Essential Functions:
1. Perform nursing duties in accordance with nursing guidelines and best practices established by the Texas Board of Nursing for Licensed Vocational Nurses
2. Perform the following duties to ensure the highest level of medical care for each resident assigned to you:
a. Schedule and attend medical appointments
b. Review charts
c. Respond to injuries and illness during normal business hours
d. Evaluate all health and medical complaints of residents and coordinate appropriate care for follow up
e. Regarding medication for assigned residents:
- Manage medication ordering and delivery
- Ensure accurate distribution of medications
- Maintain adequate supplies of medication
- Ensure proper storage of medications
- Re-label any medication that is changed with updated label
- Perform audits of MAR, as directed
- Train house managers on medication administration or any other medical function necessary including any necessary training on assistance with activities of daily living
3. Maintain medical documentation for each resident assigned:
a. Enter appointment summaries into Electronic Medical Record within 24 hours of the appointment, including any written material or oral instruction provided at appointment.
b. Update medication administration records (MAR) in real time when medications are changed, added or discontinued.
c. Create nurse progress note, in real time, in Electronic Medical Record when any communication regarding medical care is received from or provided to a resident, guardian, or Breckenridge Village staff member
d. Document health and medical complaints
4. Communicate in a clear, transparent, and timely manner all information regarding each resident assigned to you:
a. Contact the QIPD immediately when any of the following occur:
- Illness or injury requiring transport to Urgent Care or Emergency Department
- Illness that requires a medical appointment or isolation/quarantine
- Injury that requires medical attention or
- A medication change is ordered for resident.
b. Contact the parent/guardian within the same business day whenever possible but never more than 24 hours post appointment, to discuss
- Medical appointments, review documentation and instructions,
- Medication changes,
- Illnesses or injuries requiring transport to Urgent Care or Emergency Department,
- Illness that requires a medical appointment or isolation/quarantine or
- Injury that requires medical attention
5. Implement and oversee ongoing good public health practices in assigned houses and implement any guidance provided by FirstDay Foundation Chief Epidemiologist
6. Must be able to perform physical activity such as extensive walking, lifting of patients, stretching and bending
7. Report signs of abuse, neglect, and exploitation and sexually provocative behavior and implement professional response, reporting and documenting procedures
8. Adhere to professional and ethical standards of conduct outlined in Breckenridge Village personnel polity manual including demonstrating respect for agency staff, residents, and community members and complying with required dress code at all times
Measurable Deliverables:
1. Meet all state regulatory guidelines and standards that are applicable to this position, ensuring no deficiencies or concerns are found regarding medical care during annual fundamental surveys.
2. Maintain resident physical and mental safety by making immediate report of any changes in resident health of behavior to the Nurse Manager and the QIDP with whom you collaborate
3. Provide the highest level of resident care
a. Daily review of T-logs and act as required,
b. Daily review of GERS and act as required
c. Daily review of communication from residents, parents/guardians, QIDP or any other team member and act as required,
d. Address each medical concern with urgency
e. Treat resident medical needs as though you would for your own family
f. In emergency situations, call 911 first
4. Ensure proper time management of all daily, weekly, monthly, quarterly, and annual tasks
5. Document all resident health information as close to real time as possible but not later than 24 hours
6. Communication timeframes (during normal business hours):
a. Answer all phone calls immediately or call back within one hour;
b. Answer text messages immediately or text back within one hour,
c. Answer all emails within the same business day
Credentials, Skills and Abilities:
1. Maintain current licenses and certificates required for the position including, but not limited to:
a. Good standing as a Licensed Vocational Nurse with the state of Texas Board of Nursing
b. Current BLS certifications and First Aid Certification
2. Pass a pre-employment drug screen and random drug screens throughout employment.
3. Valid Texas Driver’s License.
4. Pass a pre-employment and biennial criminal background checks.
5. Must be able to maintain professional people skills, oral, written, listening, and non-verbal communication skills
6. Demonstrate the ability to:
a. Respond sensitively and competently to the service population’s cultural and socio-economic characteristics
b. Work collaboratively with other staff members, service providers, and professionals
c. Communicate effectively in writing and verbally in English.
d. Work in a fast paced environment while maintaining control and professional composure at all times and making decisions based on the needs of the program and service population
e. Maintain computer literacy required to meet the responsibilities of the position
f. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team
