Job Summary
The HIM Data Integrity Specialist maintains the master patient index (MPI) which is a critical business function and patient safety resource; it links a patient to demographics, clinical and financial information. The MPI must contain accurate, timely, and complete data that includes a single identifier for each patient registered. The HIM Data Integrity Specialist will be responsible for reviewing, analyzing, and maintaining patient data integrity within the MPI for the EHR system. This includes remediation of duplicate medical record numbers, medical record overlays and assisting with correction of any clinical and demographic information.

Work Experience Required:
- 2+ years of experience in HIM Data Integrity.
- Experience working with Epic with an emphasis on resolving duplicate medical record numbers, resolving overlays, and chart corrections.
Preferred Qualifications, Education, Licenses, Certifications, Experience, etc.:
- Associate or Bachelor’s degree in HIM or equivalent experience.
- RHIT/RHIA certification.

- Ability to analyze complex information and use critical thinking skills to determine appropriate solutions and necessary merges to protect the integrity of the EMPI.

- This role involves interaction and collaboration with other departments and requires excellent judgment and interpersonal skills.
Essential Duties and Responsibilities
- Performs medical record number merges, patient data corrections, and medical record overlay corrections in accordance with the Company’s policies and procedures.
- Processes requests for chart corrections and prioritizes all requests around patient care and safety.
- Works from reports and queues to resolve all issues in a timely and accurate manner.
- Proactively and independently solves problems and operates with a high level of accuracy by investigating complex patient identity discrepancies, analyzing multiple data points across Epic, eCW, and other EMR systems, and independently determining the appropriate corrective action (e.g., duplicate MRN merges, overlay resolution, demographic corrections) while adhering to established HIM policies and patient safety standards.
- Exhibits excellence in communication, feedback, precision, and follow by clearly documenting findings and actions taken, coordinating corrections with clinical, revenue cycle, IT, and external partners, and ensuring all downstream systems are notified and updated. Maintains meticulous attention to detail due to the high level of patient safety, privacy, and financial risk associated with inaccurate patient data and the transfer of protected health information (PHI).
- Ensure all parties involved in making corrections in any disparate systems are promptly notified and follow through to ensure all corrections are executed in a timely manner.
- Analyzes and identifies root cause of data discrepancies in the Master Patient Index and brings all issues to the HIM Manager immediately.
- Acts as key point of contact for all HIM data integrity related issues for the Company’s joint venture partners as well as other Company departments.
- May perform infrequent medical record release duties as required and participate in process improvement initiative
