Social Worker

Working in the Outpatient Adult Mental Health Clinic, you will be expected to perform assessments, develop, implement and evaluate the inter-professional plan of client care for older adults with complex mental health and addiction issues, with associated behavioural challenges, that may be complicated by physical health problems. You will liaise with other services within the Mental Health program and with community agencies to coordinate care for clients. You will be expected to utilize critical thinking, problem solving and customer service in your approach to care and service delivery. In addition, the candidate will work with an inter-professional team and participate in program development and evaluation, internal/external presentations and continuous quality improvement activities. Candidates must be capable of providing supervision to Masters of Social Work students and contributing to the development of best practices unique to the social work field. The candidate will support a workplace that embraces diversity, encouraged teamwork and complies with all applicable regulatory requirements. Ability to work flexible hours is a requirement and some evening hours will be required.

• Masters in Social Work from a recognized Social Work program
• Member in good standing with the Ontario College of Social Workers and Social Services Workers
• Formal training and demonstrated knowledge in evidence based treatment models (motivational interviewing,. interpersonal therapy etc) are critical
• three or more years of specialized clinical experience working with complex and challenging geriatric clients and their families is required
• Training and/or experience in CBT, DBT is an asset
• Strong skills in group facilitation, individual and family counselling, clinical assessment and treatment are essential
• Knowledge of community resources in mental health treatment within the geriatric sector is needed
• The ability to provide culturally competent service to clients of diverse ethno racial and cultural background is essential and experience working in a multi-system, inter-professional environment with geriatric clients is required
• Excellent organizational, interpersonal and written communication skills and the ability to work effectively in a fast paced clinical environment are also essential
• Experience in develoing and delivering educational workshops/presentations to a range of audiences and supporting vapacity building initiatives are essential
• Compuiter skills, including work processing, electronic communication and workload measures are essential
• Ability to work flexible hours is a requirement and some evening hours will be required
• Travel in the community may be required and access to a vehicle or ability to use public transit would be required

Hours: Monday-Friday - Various rotating 7.5 hour shifts - 8:00 - 16:00; (Hours subject to change based on operational needs)

Salary: $46.004 to $59.637 per hour
